INFORMATION FOR OUR PATIENTS
Welcome to Thornhill Medical Centre. We are a group of six family
physicians who are pleased to look after all aspects of your medical
care and those of your entire family. Our services begin with care
of the newborn through to caring for growing children, evolving
adults and the elderly. We have a focus on health prevention and
comprehensive care. We are well trained in caring for a broad range
of illnesses, from minor to the most serious. Community and Palliative
care make up an important part of our practice. We are pleased
to provide shared prenatal care with our local obstetricians
at York Central Hospital.
At Thornhill Medical Centre, each physician looks after his/her
own patients. If your own doctor is not available, another
doctor in our office may be able to look after your urgent problem.
Whether
you are a new patient just joining our practice, or a seasoned veteran
of the clinic, here is some information we think you might be interested
in knowing:
Services We Provide include:
- Annual Health Exams for children, teens, adults
and elderly
- Illness and Injury assessments
- Prenatal Care
- Newborn care and immunizations
- Routine Gynecologic Care (eg. Pap smears and Breast exams)
- Elderly Care

- Injections, flu shots, vaccinations

all our patient notes and encounters are complled in an Electronic
Medical Record, to assist in organizing and optimizing your health
care.
APPOINTMENTS:
We work on a booked appointment basis. We will now be using Advanced
Access Scheduling. We are not a walk-in clinic. Our Office Hours
are:
Monday to Thursdays, 8:30 am- 8 pm, and Fridays
8:30am- 5 pm.
Our nurse is available for injections, nursing needs,
advice etc. :
Monday to Friday 9:30am-11:30am and 1:30-4:30pm
,
and
some evenings from 5:30-7:30pm. (Please call during
the daytime to check on nursing availability for the evening as
needed).
We make every effort to give you a satisfactory appointment time.
We ask you to help us by briefly identifying the nature of your
problem to the receptionist when calling. This ensures that we can
prepare appropriately for any equipment or staffing to meet the
needs of your problem.
We encourage you to call our office first, before
deciding to go to a walk-in clinic. Our goal is to provide comprehensive
and timely care for our patients.
We appreciate notification if you are unable to keep an appointment
so that the time slot may be offered to someone else. There may
be a charge for missed appointments if you do not cancel 24 hours
prior.
To
avoid disappointment, please call 3-4 months ahead to book your
annual health check up ("physical") with
your doctor.

HEALTH
CARDS:
Please present your health card at every visit. It is important
to ensure that your health card is valid by checking the expiry
date.
There are some services that may not be covered by your Health Card/OHIP.

TEST
RESULTS:
We do not call patients for results that are negative/normal.
If you wish to review your results, please book an appointment with
your doctor.
If your doctor discovers an abnormal result, he/she will ask that
you book a follow-up appointment, or will give you instructions
as to what further action should be taken.

LAB SERVICES:
We do not
provide any lab services on site. Your doctor will direct you
to various labs in our area if required. If your doctor has asked
you to do a blood test, we encourage you to seek out a Lifelabs
location. We do have a direct networking link with this lab, which
facilitates accurate and prompt reporting right into our electronic
medical records. For a patient service centre near you, and hours

AFTER-HOURS
COVERAGE:
There
is always a nurse on call after office hours for our patients' urgent
problems. A doctor is available to assist when the need arises.
When you phone our office after hours you will hear a message directing
you to call the Telephone Health Advisory Service Nurse
at:
1-866-553-7205
The nurse will assess your problem over the phone and direct you.
This may involve a call back from the doctor on-call if the severity
of your medical problem requires it.
If you feel you have a life threatening emergency, please go directly
to your nearest Emergency Department, or dial 911
for an ambulance.
Saturday morning clinics are run by the on call
physician. This clinic is for urgent weekend problems only. It is
not a walk-in clinic and you are asked to book an appointment by
calling on the same morning. Call our office on Saturday
morning for information on the clinic location and how to
arrange an appointment.
On some Saturdays, our Saturday morning clinic is operated at 7700
Bathurst Street (at Centre Street), Unit 35 "Thornhill Family Physicians"
in the Promenade Village Shops Plaza. When booking your appointment
for Saturday morning, be sure to clarify the location of your appointment.

HOSPITAL
AFFILIATION:
All physicians in our office affiliated with York Central Hospital
(10 Trench Street), off of Major Mackenzie Drive in
Richmond Hill. If you are seen in the Emergency Department or require
hospital admission here, your doctor will receive a record of your
visit.

PRESCRIPTION
REFILLS:
During
your visit, your doctor will prescribe enough medications to last
until he/she would next like to reassess your medications and your
condition. For example, if you have high blood pressure and
the doctor wishes to check it every 6 months, you will be prescribed
repeats of your medication for a total of six months. At that
time you will need to come in for a new written prescription before
your medications run out completely .
To
avoid confusion about medication supply, please bring all of your
pill bottles with you at each visit. If you see that your are running
short of pills, please be sure to book an appointment at least 1
week before they finish.
If
necessary, you may ask your pharmacist to fax your doctor to request
a refill of your medication. Please allow 2 business days' response
time. We do not refill prescriptions for antibiotics in this manner.
